We take a team approach to serve patient needs and provide the best care possible. Our Care Team is responsible for delivering a high-quality, coordinated care experience for our patients at our clinics and within the healthcare system.
Leads the delivery of high-quality care, supporting the team in managing the 360 needs of the patient
Oversees daily operations of the clinic, advising culture to enable great patient care
Documents patient information, history, diagnoses, and medical decision making
Helps patients access resources to address social/economic needs and navigate the healthcare system
Ensures efficient patient flow by preparing for visit, rooming patients, and assisting the provider
Ensures patients can maximize their health benefits, helping to navigate bills and insurance
Assesses and screens patients, creates action plans, and supports transitions of care
Serves as front line for patient welcome, scheduling, and insurance verification
We deliver tailored, high-quality care through our team-based model, with more resources and support for our patients, inside and outside of our clinics. We thoughtfully design our clinics with that enable us to provide quality care in a collaborative environment. We also offer Open Houses as an opportunity to tour our clinics.

We’re a primary care provider that knows the importance of building a community of care centered around the needs of our senior patients. The Community Relations team members are focused on patient and community engagement. These roles involve building strong relationships throughout our communities to ensure that our services are accessible and well-integrated within the community.
We pride ourselves on meeting patients where they need it most. Our Community Relations team members interact with patients and community partners in a variety of settings, from Hopscotch Primary Care clinics to community events.
The Community Relations Coordinator drives both existing patient and prospective patient engagement by developing, coordinating, and managing Hopscotch events that provide opportunities for education, connection, and socialization.
As the face of Hopscotch Primary Care to the communities we serve, Outreach Leaders partner with community organizations, healthcare entities, and trusted insurance brokers to drive education, awareness, and demand to our clinics and amazing care teams.
The Patient Engagement Representative partners with our Outreach Leaders and clinical teams to ensure that prospective patient inquiries are addressed in an efficient and timely manner, ensuring that access and demand to our high-quality care is met.
