The team-based care model puts patients first and is in place with the goal to achieve better patient outcomes. I now know more about my patients. The roles like our Community Health Worker and Care Manager RN are able to bring information to me that I would not have had otherwise in a traditional setting.
I believe health care should be proactive, not reactive. I love being able to interact with my patients and discuss their goals of care. Collaborating with my patients is one of my favorite aspects of what I do on a daily basis.
Hopscotch focuses on primary care and serving the needs of the patient. It’s just not a doc visit where the doc is going from one patient to another as fast as possible.
